Lively will miss the rest of 2025 and might not pitch again until 2027. The grim report that Lively, 33, needs Tommy John surgery was not unexpected. Nevertheless, it is a blow to the Guardians.

Lively didn't factor into the decision in the first game of Saturday's doubleheader against the Red Sox, giving up four runs on six hits and three walks over five innings. He struck out four.
